Rate Increase - Effective January 1, 2012
We want to remind that you that effective January 1, 2012 and on your annual anniversary date, new pricing
will take effect. The following reflects this increase:

1. Full Coverage (ABC)
   1. Group Rate          $ 240.00
   2. Individual              $ 258.00
2. Civil and Criminal (BC)
   1. Group Rate          $ 60.00
   2. Individual              $ 64.00

Coverage Enhancements
Effective May 1, 2011 the following coverage enhancements took effect.

1. Administrative Off-Duty, Out of Scope limit increased to $5,000
2. Stand alone HR-218 Coverage limit increased to $25,000 for civil and criminal claims
