HR-218 Application

  

To submit an HR-218 Application to the FOP Legal Defense Plan, please fill out the form here. Once your application is approved, within 30 days you will be sent your FOP Legal Defense Plan ID Card, Plan Booklet, and a Certificate of Participation. If you paying by credit card, please contact Hylant Group to arrange payment after submitting the
HR-218 Application.

If you are paying by check, please make all checks payable to FOP Legal Plan, Inc. To mail your check, please contact Hylant Group.
